Prostate Health Education Network, Inc. (501c3) http://prostatehealthed.org/, 500 Victory Road Fourth Floor, Quincy, MA 02171, My position would be as a member of the Board of Directors, founded in 2003 to eliminate the African American prostate cancer disparity. Its mission also includes advocacy efforts to increase the overall support and resources for a war on prostate cancer that will lead to a cure for the disease for the benefit of all men. The Prostate Health Education Network implements these national initiatives, 1. The PHEN Survivor Network was established in 2003 and is successfully mobilizing black prostate cancer survivors to work collectively towards eliminating the racial disparity.2. Monthly Support Group Meetings and Webcasts since 2003 in partnership with the Dana-Farber Cancer Institute. The support group meetings bring survivors and their families together to learn about new prostate cancer developments, treatments and clinical trials. An educational presentation is streamed live for the public as a part of these meeting.3. The "Annual African American Prostate Cancer Disparity Summit began in 2005 to address policy, medical and community outreach initiatives to eliminate the prostate cancer racial disparity. It is a two-day program hosted in Washington, DC with live streaming.4. PHEN Online Television began broadcasting in 2007 reaching men and their families at home with educational program content developed from PHENs monthly meetings and summits and other relevant programs.5. The "Annual Fathers Day Rally" was launched in 2009 in partnership with churches nationwide who hold a special recognition and prayer for prostate cancer survivors and loved ones impacted by the disease during their Fathers Day services.6. PHENs Educational Symposiums with Church Partners began in 2013 as an outgrowth of the Fathers Day Rally. These symposiums are structured as four hour events held at the facilities of PHEN church partners in different cities with medical specialists, community leaders and the public.7. Monthly Treatments and Clinical Trials E-Newsletter began publication and distribution in 2012 to educate about evolving treatments and clinical trials to help keep the patient community knowledgeable in these areas which are critical to eliminating the disparity.8. PHENs Clinical Trials Rally was launched in 2017 to raise the awareness and knowledge about prostate cancer clinical trials to increase African American participation.9. The Daddys Boys Stage Play began performing in 2018 using entertainment to attract larger audiences for PHENs educational outreach efforts with church partners around the country. As a member of the Board of Directors I would attend their annual meeting while also advising on outreach programs, help to set new strategies, and support in carrying out the overall mission of the Prostate Health Education Network. I do not provide any investment or financial advice or make trades on behalf of this entity, January 25 2021, approximately 6 hours per month.
