Effective April 23, 2026 - Mr. Hochholzer became a member of the ThedaCare Family of Foundations Investment Committee (the foundation) and the ThedaCare Foundation-Appleton Board of Directors. The foundation focuses on enhancing each community by investing directly in preventive care, advanced medical equipment and educational initiatives that empower everyone to live their best lives. ThedaCare Family of Foundations is located at 1818 N. Meade St Appleton, WI 54911 Mr. Hochholzer is a member of the investment committee and the Thedacare Foundation-Appleton Board of Directors. In his capacity he will participate in Committee meetings to support and advise ThedaCare Family of Foundations (TCFF) staff on matters related to the finance and investment functions of the organization. TCFF employs an outside, third-party investment provider for the ongoing investment function; Committee involvement is simply in an oversight and consulting capacity. Mr. Hochholzer will devote approximately 2 hours per month to the foundation and may devote up to 1 hour per month during trading hours Participation as a voting member of the Finance & Investment Committee of the ThedaCare Family of Foundations Board of Directors. Meetings are held quarterly. Effective September 1, 2019 - Mr. Hochholzer and his wife are responsible for all operations, filings, and activities of a newly formed LLC. Winchester Park Properties, LLC 129 Parkway Drive Combined Locks, WI 54113-1265 The LLC is investment related and was created for the sole purpose of acquiring and owning a duplex/condo at 1303 Cherry St, Kaukauna, WI and will be rented to Mr. Hochholzer in-laws. The real estate and a checking account (at ASB) are (and are anticipated to be) the only assets of the LLC. Mr. Hochholzer expects to devote approximately 2 hours per month to the LLC. He is the sole member of the LLC, his wife, is the 'Assistant Manager' of the LLC to allow her to conduct business on its behalf.
