Item 11 tells you whether to look for one or more detailed disclosure answers. It does not state the nature, date, respondent, outcome, or current relevance of an event.

Open the Disclosures card and match each event narrative to its Yes row. Record the respondent, date, forum, allegations or findings, sanction or outcome, and current status. If an individual is named, compare the name with the current advisor roster.
